2026 Cloverbud Day Camp Registration
The dates for the 4-H Cloverbud camp are June 4 & 5 from 9:00am to 3:00pm.  Cost is $45 (*candy bar discounts may be applied)
The deadline to register is May 15th. Please click here for registration form

Additional forms that you may need for 4-H Camp: 
*Ohio 4-H Camp Health Form WILL BE MAILED AFTER REGISTRATION - required for EVERY camper coming to a 4-H camp!
Perry Co. 4-H Junior Camp - Camper Code of Behavior - FOR YOUR REVIEW (signed with online registration)
Restricted Release - for legal/safety restrictions on who is able to pick your child up at the close of camp
Immunization Exemption Form - for campers who are not up to date on school-required immunizations listed on the camp health form
Dietary Restriction Form - for 4-H Camp Ohio, if any special dietary accomodations need to be requested - MUST BE SUBMITTED PRIOR TO CAMP!
